We are going to talk a lot about toothaches because I have a toothache expert in, Dr. Irvin. How are you Dr. Irvin?

(Dr. Irvin) Good, good. How about you?

(Dr. Marvin) I am doing well. Thanks for joining us today. We have a great topic. People don’t know that not all dentists are 9-5 dentists, like myself. I have at The Center for Natural Dentistry; we are opened 4 days a week for about 10 hours each day. And what we do is we help people. But what happens after hours? What happens on the weekends? What happens after? If you an existing patient obviously you can call us and you will get either myself or Dr. Godes. But if after hours if you don’t have a dentist and you’re not a patient of record in our office where do we send you? We’ll let me tell you about this. There is a new office in town. Been here for almost a year, and Dr. Irvin actually comes from that office. That office is actually called On Demand Dentist, fitting isn’t it?

(Dr. Irvin) Yep.

(Dr. Marvin) What they do is when you need a dentist and you need one immediately, which is what On Demand Dentist is there for. It’s kind of like an urgent care for dentist. Why don’t you tell me a little bit about what On Demand Dentist is?

(Dr. Irvin) Well, On Demand Dentist is a practice and we have a bunch of dentist that are actually on call 24/7. So basically if you have any toothaches, or tooth pain, or anything of that nature you want to give us a call and one of our dentist will come down there and help you out, help you get out of pain.

(Dr. Marvin) So it’s for people that lets say haven’t been to the dentist in a while and they have excruciating pain, right?

(Dr. Irvin) Yes, definitely.

(Dr. Marvin) Ok so, how do people find you?

(Dr. Irvin) So on the, they either go online and they Google search ODD or On Demand Dentist or emergency dentist around the area. We are located in San Diego, Claremont Mesa.

(Dr. Marvin) Great. And then so they have a toothache, can you tell me you know what’s the difference between your practice and just a regular dental practice?

(Dr. Irvin) Well, On Demand Dentist basically again is 24/7 its all the time so a regular dental practice you would call in and they would try to fit you in to the schedule, which is really packed, and the dentist is working constantly bouncing between patients. Really a high rate practice. Here the dentist is available at all times so you call us you get an appointment scheduled, which is usually right after you call us. We come right down there and help you out. So you are the first person in there and you get the one on one service that you deserve.

(Dr. Marvin) I think that is important because a lot of people first of all don’t understand that when you have a toothache that really consumes your whole life.

(Dr. Irvin) Exactly.

(Dr. Marvin) You can’t eat, you can’t sleep, you can’t go to work, and you can’t think about your family, you can’t enjoy life. I mean a toothache if you have ever had one can seriously control your whole life. And On Demand Dentist is there to alleviate people’s pain when they need it most. So when do people call?

(Dr. Irvin) People usually call on the weekends, at night, I would say Thursday night on to Sunday night is pretty much the times where dentist love to take the weekend off in their comprehensive practices.

(Dr. Marvin) Yeah I think most people don’t understand that dentist like the comfortable 9-5 job. Most dentists only work 4 maybe 5 days a week. We only work 4 days a week, 4 long days, but they get tired and they want their time with their families or their free time but what I find in my practice and the practices that I worked in the past and other dentist that I talk to is that these toothaches ok, that people have they usually come after work. They come at the end of the day and what do you think? Do you think most dentists, you know, want to see an emergency patient?

(Dr. Irvin) Defiantly not. This is exactly where we come in. I mean On Demand Dentist we actually love it. I love the fact that I am on call and I can help relive somebody’s pain. Because toothaches happen sporadically. They just kind of pop up.

(Dr. Marvin) Yeah toothaches can come and go and I don’t know if you have ever had a toothache before, you know it can come and it can go. And the first thing is how do you relive the pain? Well, there are over the counter ways or at home remedies that you can use to alleviate the pain, but one thing that I always talk about on this show, Let’s Talk Dentistry, is that you deal with the cause of the problem not just deal with the symptom. And pain is your body’s way of saying that there is something wrong. Now you can alleviate the pain by taking your pain medication or you can alleviate the pain because it the nerve that is in pain, you can take out the nerve and that’s what a root canal does. But just like I talk about all the time, if you are not addressing the root cause of the problem you’re really just putting off a symptom until its worse where the pain comes back even stronger with vengeance and since pain is a motivating factor you know On Demand Dentist can really seriously help people, especially when they need it most. And a lot of times these happen in the middle of the night pain can come on for no reason at all. One of the things that you’ve you told me Dr. Irvin is that the pain that these people are experiencing from a level of 0 to 10 what is the pain level for these patients?

(Dr. Irvin) Average 8 or so, it’s pretty high.

(Dr. Marvin) 8, some 9s, probably 10s. One of the comparisons from my patients when they talk about pain or tooth pain, more specifically, especially from my female patients of course it’s that the pain is so excruciating its worse than child birth. That’s some of the pain. So obviously you and I haven’t experienced that but that, you can imagine having a child you come out of your body versus a toothache and these people are saying that the toothache is worse. So if you are there with they need it most you can be pretty much have a big S on your chest and you’re like a superhero to these people aren’t you?

(Dr. Irvin) I guess you could call it that.

(Dr. Marvin) So what are some of the procedures that you do to help these people get out of pain?

(Dr. Irvin) So basically right when you come to the office we would take an x-ray and do thorough toothache exam on you. Then we would go over your options after that. So we have the x-rays that we actually print out and let you see in hand. Everything is right up front. There are no hidden fees. It’s completely honest 1 on 1 time. So your options after that assuming we can localize the tooth and we can definitely tell you where the pain is coming from. We basically tell you our options are extraction which is taking the tooth out or a pulpectomy which is removing the nerve. We do not do complete root canals but we can get you started, if the nerve needs to be taken out if the patient is scared of the actual tooth being taken out.

(Dr. Marvin) Ok, so you what would you say your number 1 procedure is for a toothache?

(Dr. Irvin) Oh extractions.

(Dr. Marvin) Extractions. And if don’t know what an extraction is it’s where you take out the tooth. And one of the procedures that you mentioned is a pulpectomy. And that, you mentioned that this is not the root canal so let me explain what a pulpectomy is for people that don’t know what a pulpectomy is. A pulpectomy is literally the first step of a root canal. When you have damaged the nerve or let’s say you have a toothache and the nerve is throbbing, one of the things that you can do, or one of the procedures you can do to help get you out of pain is to take out the nerve. Well this procedure to take out the nerve is called a pulpectomy. Can you tell me Dr. Irvin, what’s the difference between a pulpectomy and a root canal?

(Dr. Irvin) Pulpectomy is basically, you know you open up the top of the tooth up and you go down there and you remove the nerve, and that’s it. You literally seal the top of it just to get the patient out of pain so that pain is from the nerve basically toggling like a little pressure switch causing the patient striking pain or even dull and achy pain. We would remove the nerve and that’s it. And let the whole relief happen.

(Dr. Marvin) So what happens to the patient after we remove the nerve? They are obviously out of pain but what’s next?

(Dr. Irvin) They are out of pain but at the same time if they want to move forward with a regular root canal they would have to go to a comprehensive dentist and then do it there.

(Dr. Marvin) Comprehensive dentist or an endodontic, so what is the remaining part of a root canal? What is left to do after you take out the nerve? Because some people think that a root canal is simply they don’t even know what a root canal is, so what is the rest of the root canal?

(Dr. Irvin) The rest of the root canal is cleaning out the canal after you take out the nerve, irrigating it with some special solution basically to clean out all the bacteria in there, and then filling it with a rubber material called guta percha, and then putting a post in it and then putting a crown on the tooth. So it’s a lot of work to go into a root canal further than just going into the nerve.

(Dr. Marvin) So why do you have to put a crown on the tooth?

(Dr. Irvin) Basically in order to seal the margins. Once we drill into tooth you want that margin to be sealed so no other bacteria gets in it after a root canal if that was the choice. Yeah, you just want the margins to be sealed in order for that tooth to be wholesome, so to speak.

(Dr. Marvin) Ok. So back up. We have taken out the nerve, that’s called a pulpecotmy, where you take out the nerve. Then you go in there and you clean and disinfect the canals. Now I am going to tell you as a holistic, biological dentist clean the nerves and disinfecting may work for a little bit but it doesn’t get into all the canals so I personally have some views again root canals and therefore it is not a procedure that we endorse but I do endorse it for people that for a particular instances where A you need to have the tooth and you need to get out of pain. Let me give you an example. If someone were to fly in from our of country and it was their front tooth that hurt and we don’t have a solution to they have a front tooth that is in pain and removing the nerve will get them out of pain and also have them keep the tooth at least temporarily then I would endorse that procedure. You agree?

(Dr. Irvin) Definitely. I agree.

(Dr. Marvin) One of the reasons why I like Dr. Irvin, he is actually works at our office as our sleep medicine specialist at our office. And the reason why I like him is because I have been sharing and mentoring him a little bit about these root canals and you know, at the On Demand Dentist you do clean out the infections and do the first step but do you fully endorse root canals?

(Dr. Irvin) Personally, I do not. I not a big fan of just cleaning something temporarily. I like the more definitive treatment and this is exactly what On Demand Dentist is about, and it’s the way I live.

(Dr. Marvin) So another thing about root canals is once they are out of pain and they go home, what usually happens because when they go to the dentist or if they go to the dentist afterwards are they hit with a shock or a shock at the price tag for finishing it.

(Dr. Irvin) Oh yeah definitely. They just think it’s a quick fit. A cap should always cost a very minimal amount to patients when actually crowns can run the cheapest crown I have seen is $700 and it can run up to $3,000 maybe $4,000. Plus the post.

(Dr. Marvin) There are plenty of procedures that need to happen and a lot of money that has to be invested into a tooth. And one thing that is important when you decide to do a pulpectomy, and we don’t you know, pulpecotmy are not done very often because most dentist go straight forward and do the full root canal but when a root canal is started a pulpecomty is done there is still a lot of procedures and a lot of money that has to go in. So Dr. Irvin, when you talk to patients about the pulpectomy you also talks to them about the next steps and the cost it involves. And people in order to save these teeth literally have to spend 2, maybe $3,000 just to save the tooth and you know. There has to be reason why people come to On Demand Dentist because they haven’t been to the dentist.

(Dr. Irvin) Exactly, exactly.

(Dr. Marvin) And if they haven’t been to the dentist they sometimes they haven’t had a root canal before and crowns and so this is where the thing about dentistry can get very expensive. This is that dentist are expensive is because the procedures are expensive and a lot of people just don’t like going to the dentist. Am I right?

(Dr. Irvin) Exactly, exactly.

(Dr. Marvin) So if you don’t go to the dentist, ok. Then you could have obviously a toothache and you could have a cavity I would say most toothaches are caused by cavities that have gotten too big.

(Dr. Irvin) yes. Deep cavities close to the nerve.

(Dr. Marvin) Close to the nerve. And don’t a lot of people think that they can just put a filling or get a crown on the tooth?

(Dr. Irvin) They do think that, but again after our assessment and our exam and our x-rays, usually the x-rays show that the decay is right on the nerve. This is the reason why they are having pain. It is striking its actually nerve in your jaw bone so.

(Dr. Marvin) Yeah and you said that you take x-rays right?

(Dr. Irvin) Mmmhhmm.

(Dr. Marvin) So what do you see on the x-rays on these teeth that have tooth decay?

(Dr. Irvin) Well what we see in dental terms blown out is the basic way I can say it, but you would have a basically a dark spot in place where it was supposed to be white in an x-ray, lay men’s terms but,

(Dr. Marvin) that is one of the things that at The Center for Natural Dentistry, we have what’s called a 3-D x-ray, a cone beam x-ray. Do you have one of those at On Demand Dentist?

(Dr. Irvin) We do not. We have the panoramic x-ray, the one that goes around your head.

(Dr. Marvin) Ok, so keep in mind that at On Demand Dentist there is just a panoramic x-ray, at our office The Center for Natural Dentistry we take we call it a cone beam or a 3 dimensional x-ray where we get to look at the teeth from multiple angles and actually can see those holes a lot clearer and one thing about x-rays it that it only shows you what it can give. And it’s up for interpretation. So what your actually seeing on the x-ray when you have a lesion or you have a hole at the end of the tooth is that part of the bone has been eaten away by the infection and one of the reasons why we don’t do root canals is because when you do a root canal it does not clean out that infection and it doesn’t fill in with health bone and so one of our focus at The Center for Natural Dentistry is to focus on more definitive treatment which is to not only clean out the infection but sometimes we have to take out the tooth. Is that something at On Demand Dentist, you know, do you do that also?

(Dr. Irvin) Filling wise, no we don’t if that’s what you are getting at. We do not do fillings. Basically, I want to say retract a little bit on the panoramic x-ray, on an x-ray in general you only see 35% of what is actually there. So if something is close to the nerve in terms of the x-ray you are in the nerve and therefore it easier has to come out or take the tooth out.

(Dr. Marvin) That’s a good point because on an x-ray if you see something on the x-ray that means that the process has been going on for quite a while so in dental talk Dr. Irvin and I can be like yeah you see a large cavity on an x-ray but in real life when you take a look at it with magnifying glasses and we look then the cavity is a lot deeper and so if it’s a lot deeper that means that bacteria has actually gotten to the nerve traveled down to the nerve traveled down through the root gone outside of the tooth and one of the clinical signs we see is that if a tooth hurts when you bite down on it, ok. Chances are that you have an infection at the end of the tooth. And if you have an infection at the end of the tooth again cleaning it, doing a filling, or cleaning out taking out the nerve is not going to solve the problem. And so this is very interesting because we want to make sure that we get to the root cause of the problems that’s what The Center for Natural Dentistry does. Dr. Irvin at On Demand Dentist takes care of the root cause too by taking out the tooth and also taking out the infections especially when the tooth aches are pretty bad. So Dr. Irvin tell me about what On Demand Dentist does not do. Because you mentioned that you help people with toothaches. What about people who don’t have toothaches?

(Dr. Irvin) Well the people who without the toothaches we basically do not do any cleanings, we don’t do fillings, we don’t do crowns, we don’t do teeth whiting. So we are strictly, strictly, strictly toothache relief.

(Dr. Marvin) So I can’t come see you for a checkup?

(Dr. Irvin) No definitely not.

(Dr. Marvin) Ok. What about if I wanted dentures?

(Dr. Irvin) No dentures as well.

(Dr. Marvin) Implants?

(Dr. Irvin) No implants.

(Dr. Marvin) Bridges? Crowns? Anything?

(Dr. Irvin) No, no bridges, no bone grafts we just basically get your out of pain and get you on your way.

(Dr. Marvin) Ok so it’s kind of like you are going to the emergency room. You don’t go to the emergency room for a checkup or for someone to just draw blood and see how a physical or anything like that. So that’s an interesting concept. So you are there just to help people basically get out of pain?

(Dr. Irvin) Mmmhhmmm.

(Dr. Marvin) Ok, so at our office at The Center for Natural Dentistry of course we do a lot more procedures. We do take care of people’s toothaches but one of the things that we focus on is getting rid of the source of the problem which can be an infection but also restoring the teeth with good materials maybe it’s a plastic material. Of course we take out toxins that are found in people’s mouths such as mercury fillings, or let’s say it’s a metal crown or maybe it’s an infection around a root canal or metal braces or anything like that so our office is a little bit different kind of tailored more toward people that are more holistic people that want a healthier approach that want a more common sense whole body approach but you know, we help people this way but I am going to tell you, Dr. Irvin, you help people in your own ways right?

(Dr. Irvin) Exactly.

(Dr. Marvin) So tell me what the typical, or tell me a story of someone that went to the office and you know that basically got what they came for.

(Dr. Irvin) Ok, well I guess one of the most common patients that come by is ones that call for an appointment and they say that they were referred from urgent care. I am saying well basically right here at On Demand Dentist go check for us online so you can bypass the whole emergency room or urgent care. They are just going to send you to us anyways. So a patient is highly in pain, the other day I had on an upper right tooth he couldn’t bite down at all couldn’t open his mouth.

(Dr. Marvin) So he couldn’t eat?

(Dr. Irvin) He couldn’t do it; he couldn’t eat and couldn’t do anything. He was really swollen. He was really surprised that at our panoramic x-ray because there is nothing that you have to do to bite down on anything to take an x-ray which is awesome to patients and it’s like a little ride at Disney Land or something. They just kind sit and have their picture taken.

(Dr. Marvin) What I want to tell people is that if you are used to having x-rays done where they put something in your mouth, its either a piece of film with a little piece of paper or it’s a digital sensor. It’s a little bit thicker than film and it has a wire on it you know it can get pretty uncomfortable when you put it into peoples mouth and it could be even more uncomfortable when their mouth is in pain they can’t open their mouth and so this x-ray you don’t have to do that so you don’t really have to sick anything in your mouth to make it uncomfortable then?

(Dr. Irvin) Nothing goes in.

(Dr. Marvin) Alright, so that’s one benefit for going to On Demand Dentist. What else? What happened next?

(Dr. Irvin) Basically so I gave the patient his options and we were going to take out the tooth. He was ok with that he just wanted to get out of pain, so right after we anesthetized basically numbed him up he automatically started feeling better. Took out the tooth, it took about 10 minutes I would say. Popped the tooth right out and he automatically, like you could tell the expression on his face he was so like so much better. Like his livelihood just came back.

(Dr. Marvin) yeah I think its funny when you said that when you numbed him up, because a lot of people their scared of getting a shot. But these people it’s kind of the opposite. They are in excruciating pain without being numb and then they can’t even wait to get the shot or to get numb because the pain will go away the moment that the anesthetic kicks in.

(Dr. Irvin) yeah exactly.

(Dr. Marvin) Isn’t that pretty funny how, how things can change if the circumstances.

(Dr. Irvin) yeah, all your fears are out the window.

(Dr. Marvin) Yeah so you popped it out and you said that he was a new man. How would you explain that?

(Dr. Irvin) Oh yeah. The livelihood came back. I mean literally the color in his face changed. His eyes kind of cleared up. I scrapped out the infection and gave him some sutures, some stiches and he was happy. Just completely happy that the one cause of pain that had been a problem for him, he said it was problem on and off for a couple months. So now the main thing was just getting him to the dentist and now we are able to take care of that. He is good to go.

(Dr. Marvin) That’s a long time a couple of months of dealing with pain. Just imagine if you’re out there listening to this show and you have pain in a tooth. Maybe it comes and goes a little bit there but if it’s been a problem I have got to tell you pain in your body’s way of saying there is something going on. And if this has been bothering him and it kept escalating and it continues to get worse and now the pain is so bad that he has to call in the middle of the night or something like that and have to bring in a dentist in order to do that. It has to be pretty bad.

(Dr. Irvin) Oh yeah.

(Dr. Marvin) I expect, you know correct me if I am wrong. If you, was he a big guy?

(Dr. Irvin) yeah he was.

(Dr. Marvin) Ok. He’s a big guy, going to tough it out, can handle a lot of pain. But eventually he couldn’t handle this pain. So what we did, so you ended up taking out the tooth and he feels a lot better and the best thing about it is that a you were available you took care of his pain you didn’t just patch it up though. And so what do you expect from him moving forward? Do you think that, you know, is he going to be hurting afterward?

(Dr. Irvin) No. His pain is going to be less than before basically. So for about a day afterward you would be a little sore but there will be no striking pain like before he came in.

(Dr. Marvin) So that is pretty much expected. You know you have a surgery where you have to extract a tooth it’s kind of like if you had a fall and you had a bruise or a cut in your knee or elbow or hand something like that it’s going to be sore and throbbing for a few days right?

(Dr. Irvin) Yeah. So that’s pretty much explained. That’s a very cool thing that you are doing. As tying it in to natural dentistry and what we do at The Center for Natural Dentistry this kind of fits in because we realize that there are a lot of people that don’t go to the dentist and your listening to this show and you haven’t been to the dentist for a while and you don’t want to become overwhelmed by all of the comprehensive exams and hitting you with all these things and you have a toothache. You can literally call you know Dr. Irvin and On Demand Dentist and you guys aren’t going to bombarded people with all the treatments and stuff like that.

(Dr. Irvin) Oh no. We tell you what you need; right there just to get you out of pain.

(Dr. Marvin) And so it’s a great concept and I am glad that your around I have got to tell you that if I have a patient that calls our office I am going to call you especially if it’s in the middle of the night, if I am on vacation, or if Dr. Godes because we do not as a patient at our office, as an existing patient at our office we don’t prioritize an emergency patient over our existing schedule. We only see one patient at a time. I am going to tell you that most dentists are like this. They do not want to ruin their existing schedule to see new patients. So if you want to be you know get the attention that you deserve if you want a dentist to look solely at the issue at hand and not tell you about all these other stuff that you have to do in your mouth. A really large infection. Tooth number 3. Upper right first molar 6 year molars. One of the biggest teeth in the mouth and you will see an infection and you wouldn’t see this infection very clearly on a regular x-ray, the x-ray that most dentist do. So I took a picture of what it looks like on a 3 dimensional x-rays so you can see the large infection. This infection was also located on the thyroid meridian and she has been told that she has thyroid problems, and it is also located on the breast meridian and this is on the right side of her mouth she has a lot of lymph node issues around her arm pit area which is right along the breast area which is where this tooth is also located on so it’s very interesting. So let’s talk about discoloration of teeth. I got this one patient this week that came in with one and we got a question on drmarvin.com about discoloration. So let’s say that you have a tooth that is changing colors compared to the neighboring teeth. What caused it? A lot of times and sometimes and people just don’t know why it happened but it is a sign or symptom. If you were a member every single week I talk about signs and symptoms how it can drive you toward doing treatment. A sign and a symptom is like pain, pain when you drink something cold, pain when you are biting down on something.

Well discoloration is a sign that something is happening or something has happened so this one patient that actually came in he had discoloration on a front tooth and he mentioned it to the dentist and the dentist didn’t know about it, and he goes hey now I can see that its discolored lets go ahead and take an x-ray. So that sign spurred this dentist to take an x-ray and when they looked at the x-ray they saw something funny going on at the end of the tooth. What they are looking at one a dimensional x-rays is to see if there is any pathology or any changes in the bone at the end of the tooth. Now there are no other signs for this tooth other than the color because he is not having any pain. But what they are recommending is they are recommending a root canal. Well my question is always what caused the discoloration.

The first question that I asked him was have you had braces in the past and the reason why I could tell is because he had some missing teeth. And if you have missing teeth in the bicuspid or pre molar area where they are like the back teeth but they are smaller back teeth. If you have had teeth removed then I know you have had orthodontics, because orthodontics recommended them to remove those teeth when he was either a teenager or even earlier. And the reason why they do that is because they notice crowding and since they notice crowding they pull out these teeth. The problem is that these are perfectly fine teeth and they are removing them at an early age when the body still has not grown to its full potential. In other words the jaw is not big enough to have all the teeth. So what orthodontists do is they recommend they extract these teeth so that they can close up the spaces with the other teeth saying that your teeth are too big and your jaw is too small. The problem is that they don’t understand what happens to these teeth because when you, when you pull the teeth together you make your arch smaller and your remaining teeth are not lined up directly over the jaw bone and when it’s not directed directly over the jaw bone when you move the teeth you can be harming 1 or 2 of these teeth and you could be causing 1 or 2 of these teeth to die. And that’s where this young man had the tooth die. And since its dead meaning that the nerve is dead and the arteries is probably pinched or the nerve is pinched therefore the tooth is not alive and it starts to stain from the inside of the tooth out. So you can have trauma to this tooth and this particular case it was because of the orthodontics and other cases like the one case that I have online is that they this person had an accident they hit their front tooth and because they hit their front tooth the tooth did not show any signs at the time, but over time the tooth nerve or the arteries was pinched and over time the tooth started to die.

So figuring out the cause of the problem is more important at this point, because it will help decide but this one dentist or what most dentists is that if you have a discolored tooth something rings in their head that oh there is treatment to be had. So what they have done is they recommended a root canal. The problem with this is not only an aesthetic problem but there damaged the tooth by doing the root canal by drilling a hole in back and cleaning out the tooth but they are not going to dealing with the infection because they know that you won’t get any pain after cleaning out the infection because the goal is to keep the tooth from getting darker. Doing a root canal and trying to stain, bleach the tooth or to remove the stains inside the tooth so that it matches the other teeth. But if you listen me before I am not a big fan of root canals because it does not get rid of the infection that is outside of the tooth. Now all the dentist cares about is the aesthetics and the money.

So if you, that’s all they care about they are going to recommend treatments because this is what they are used to this is what they have been trained to do Andy’s dental schools. So figure out the cause tried to deal with the infections and if you want you may not need to bleach or do a root canal inside the to see if that is the best option for you so if you have a discolored tooth and you don’t know what to do it can vary depending on the cost and depending on your vanity on the color of the juice. So the first thing is that we deal with the infection. Infections are the most important thing. So in our office we have different options. One option is using ozone therapy. There are different delivery techniques in order to clean out the infection or delivery of the ozone so you have to come in and we have to figure out and we have to test. A lot of times these teeth are on the adrenal pathways and these adrenal so are your energy. It’s your fight or flight has very specific hormones that go throughout your body your adrenals control they have the adrenaline and also the court is all.

So very important. If you have an infection we need to determine if it is affecting your adrenals it’s also on your uterus on your prostate on your bladder on all of those pathways so figure out the problem figure out if it is affecting you and that will help determine what the treatment is. See how we do this? We figure out the cost we figure out alternative safe for treatments we get to the root cause of the problem with these treatments and we can help your overall health by targeting or so or figuring out the cars as opposed to just dealing with the symptom. That’s the kind of detective work that a very good biological dentist can do. So if you have a discolored tooth hopefully you learned something today and I encourage you to come in for a visit for a simple exam and x-rays noninvasive and figure out what the best treatment is for that problem. (Anne) Dr. Marvin you talked about your thyroid problems you have and all the symptoms I have. Being so tired, and so forth and you said that you got the proper medication and you said it makes you feel just swell.

(Dr. Marvin) Yes.

(Anne) I was wondering what is it you are taking or

(Dr. Marvin) Ok. Um, it is a prescription.

(Anne) Yes.

(Dr. Marvin) Ok. And it is not a synthetic medication. Its actually real thyroid gland, ok. Because it, I have a low thyroid output or my thyroid isn’t functioning properly

(Anne) Mine too.

(Dr. Marvin) So I am taking a natural thyroid product and it basically supplements so that my body functions better because of, you know I am taking the right amount of thyroid and my body is using the right amount of thyroid. It actually called Nature Thyroid.

(Anne) And what are the natural ones you are taking?

(Dr. Marvin) well they come from a different, this one I believe is a pig thyroid.

(Anne) How do you spell that?

(Dr. Marvin) It’s a pig. It comes from a different animal. It’s a very very small amount. You have to go the right practitioner to know how much to take because it’s a super small amount, I don’t even need water to swallow it. It’s that small. And I take it every single morning. And I was reluctant to take it and I have very strong views about supplements ok. You need to figure out; I don’t want to be dependent on anything, ok. So I take minimal amounts of supplements and the goal is that you only take enough in hopes that your body won’t become dependent upon it. And your body will start functioning and producing your own that I will be off. But it’s very, it’s a very sensitive hormone and you want to make sure that you get the right amount. And it’s for people with lower thyroid function, not with too much thyroid function.

(Anne) Yeah because I take thyroxin and my life, I have problems sleeping and so forth and when you sound like you sound so much better I have been meaning to call you. How can I, who did you, go to get this medication? Or this answer?

(Dr. Marvin) Yes. This particular, there are a lot of doctors a lot of integrated medical doctors that understand this. You first, the doctor that I go to he is located in Hill Crest area. His name is Dr. John Huminston, Huminston.

And he has a theory on why I have a low thyroid. So that is figuring out the cause, very important so you can minimize the damage so he was able to dose me to figure out what we should start out on. And I got to tell you, it’s night and day, and these glands are so small but they regulate a whole bunch of things going on inside your body. So I encourage you. Let’s get to the topic of cavities. Ok. Cavities are really by definition is a hole. A civility a hole in your tooth, now that means that you need to figure out why you have a hole in the tooth. Now there are holes in people’s teeth for genetics. You could have grooves, grooves, and pits, pits in your teeth that can collect bacteria and stain and those can be considered holes. You can also develop holes from certain grinding wear down on your teeth, from clenching from dentist from fillings being removed or coming out so there are many reasons why you have holes. Most people think that cavities start because of decay and that is one way you can have cavities. So a cavity does not mean that you have decay, cavity means that you have a hole but you have to figure out why you have a hole because some dentist say you have a cavity you have to fill it but is that normal. Is your hole going to get worse?

That’s the best question that you need to ask. And that’s where you have to figure out if you are cavity prone or decay prone. What is your diet like? That’s why we always go back to nutrition and diet. Is your diet more conducive to a hole getting bigger? You also have to evaluate the hole. What side of the tooth is it on? Is it along the gum line? If you have a hole along the gum line is that now considered a cavity? Yes. Or is it now going to be more prone to decay and should you take care of it now and in the future a cavity on the side of the tooth could be because your tooth is flexing. And if your tooth is flexing the enamel is flaking off. And if the enamel is flaking off you have to ask why the tooth is flexing and why the enamel is flaking off. Because if you fill these holes let’s say with a composite or a white filling, your white filling might come off because you have not addressed the root cause. A lot of these root causes are because of the bite, that’s why we look at everything.

This is very important you need to figure out the cause of the problem because any treatment could be failed, could fail because of getting the wrong cause. I hope people are clear that cavities are not necessarily decay but they can be caused by decay. Also these cavities and if there are decay in there they can be arrested or stopped or kept from getting bigger or deeper by using more different techniques other than a filling. See most dentists only have one way to fix things and the way the one way that they fix things is through fillings, why? Because that’s the way we were taught and that’s how they will get paid and because that’s what insurances pay for. See I don’t dictate my treatment based on what insurance pay for. I hope you don’t either. For instance if you, if your insurance if you have an infection in your finger nail and your insurance pays to cut off your finger would you do it? Insurance pays for it. Would you still do it? Absolutely not because you want to take care of the infection not but you don’t care about but you care more about saving your finger.

So that’s what we are talking about if you have a cavity go to a dentist that can properly diagnosis and put a treatment plan together to prevent any decay from getting worse, or you can just live with that cavity and save money that way. People think that holistic dentistry is more expensive, actually it’s cheaper because we are not doing any damage to your teeth if we are going to use a more natural treatment or we can prevent problems by just a little bit of knowledge that we provide and providing you with much better advice. This is important. I want people to understand that holistic dentistry does not have to be expensive, does not have to be painful and it can revolve solely on the initial diagnosis exam and treatment plan. It’s a very popular topic that I get asked all the time. People get their teeth stained, unfortunately they get their teeth stained they don’t like the way their teeth look and so they contemplate whether to whiten their teeth and they want to know if it’s safe and what are the possible complications, how much things cost, things like that. So I will get to that, I will also get to dental implants. I’ve had some great questions on dental implants on drmavin.com and so I am going to get to that. I am going to talk about symptoms vs. cures, ok.
Curing the problem or curing the cause versus curing the symptom. Some stuff I have talked about before but I just want to get this crystal clear with a lot of my listeners. So that they understand where the dents is coming from and I also am going to talk about health versus looks. Ok. Are you vein or are your health consciences? You can be both, but rarely can you be both. You have to make a decision between the two. And this is what this show is all about. This show is all about information, knowledge, and you making the choice whether to take this information and apply it or just take this information and put it into your knowledge base in order to make better decisions. And this is what I am all about. I am all about; I wake up early Saturday morning. My son, 3 year old son woke me up, ok. Wants to play but instead I have to change, get ready, go to the car and talk to you about dentistry. And this is what I enjoy. I don’t have to do this but I do this because I want to help you. So your listening to Dr. Marvin this is Let’s Talk Dentistry. (Sneezes) Excuse me. That needed to come out. Excuse me. You’re listening to Let’s Talk Dentistry with Dr. Marvin. I am a licensed dentist here in California, who does dentistry with the whole body in mind. With the health in mind. In other words doing what is best for you, not just what is best for your teeth.
Now I am going to talk about, about this person from Dublin Ireland, but I do want to mention this is very important. I do want to mention that a lot of what I say other dentists hate. Ok. What I say is very controversial. What I say a lot of doctors, a lot of people just don’t want me to say anything. They want me to shut up. They want me to not tell anyone about this because they say that I am quackery. That this is not true, this not based on science, which it is based on science. That it will ruin the profession of dentistry and most importantly they think that I am just out to get people’s money and to deceive people. Now there are always many ways to look at things, but if you go to our website and you look at our view on root canals which is one of the more controversial topics that we talk about. There are dentists that go on to our website and comment and say that it’s a disgrace when dentist like myself disgrace the profession. And the say that this is “information that is false, not based by science” and that I am trying to deceive people. Well let me get this straight.
There are many different types of people but two particular types of people out there. There is one particular person that likes the information that I have then there is another person that doesn’t’ like the information. If you don’t like the information that I have then don’t come and see me. You don’t come to The Center for Natural Dentistry, don’t see Dr. Godes or myself and you live your way. You go find a dentist that is in alignment with what you believe. Now I believe in common sense dealing with the root cause of problems, ok. Being proactive and being health oriented. I believe that the whole body is interconnected and what you do with the mouth affects the whole body so that’s what I believe. If you are on those, that plain of thought if you believe in the same things like common sense, like if you have an infection in your mouth can cause an infection elsewhere in the body so you take care of the infection in the mouth then you do that. If you believe that mercury is a known toxin in this world, that’s why we don’t put it in thermometers, or barometers, we don’t put it in batteries anymore because we don’t want to pollute us. And we don’t play with it in chemistry class anymore because we now know. If you know that mercury is a poison then why are we putting it 2 inches from our brain? Why are we putting it in our mouth? See this for me is common sense. You know you don’t need to have hard science to prove that mercury is bad, so you don’t put it in people’s mouths.
So what we do, or what we talk about is more common sense we would like to deal with the root cause of problems we just don’t just cover up symptoms. I deal with people a lot, with tooth pain and see pain coming from a tooth means that there is something going wrong. Your body is speaking to you. Your body has been designed to tell you something is going wrong. It’s why we have pain. Now if I took something, let’s say I go to the store, could be a drug store and I just pick up an Advil or a Tylenol. And if I take the Tylenol and the pain goes away, pain is the symptom, pain goes away did I really fix the problem? Did I really treat the cause of the pain or did I just cover it up. You see a lot of people out there believe that if you don’t have any symptoms or if you suppress the symptoms then you are ok. I am not one of those people, and I hope you’re not one of those too. So if you deal with symptoms all the time, you get this all the time. I have a root canal but I have no problems with it. I don’t have any pain, ok, so therefore there is no problem. It’s what a lot of dentists and endodontic who are root canal specialist say it’s a success so that word success is relative. It’s relived because they think success as we saved a tooth, it’s still in the mouth, and there are no symptoms so therefore it’s a success. To me it’s not a success because you can have an underlying problem. And if you have an underlying problem and there is no symptom the problem is still there, you need to go to a dentist that understands that there could be something. So that’s my type of listener, that’s my type of patients that who I am and those are the people that I service. So we are not everything to everyone.
Our office does treatment, we are for the people that are health conscience that are aware of toxins that know that the body is connected that have common sense and are more proactive about their health. So if you are not one of those types of people then you can listen in but you don’t have to come see us. You don’t have to get an evaluation. I am not, I don’t have enough time to treat everyone in this world but I am, I do have enough time to share this information to either chose you in or chose you out of becoming a patient.

(Dr. Marvin) Ok. Great. What’s your question?
(Female voice) I know that you’re not an orthodontist but I hear so much about how bad the metal is to have in the mouth.
(Dr. Marvin) Ok. So you want my opinion?
(Female voice) Any recommendations?
(Dr. Marvin) Opinion about orthodontics in general or the braces?
(Female voice) Yeah my 16 year old needs braces so you know I am just wondering what I should request, or who would be the best, you know what I need to look for in an orthodontist. And you know, what I would request for braces?
(Dr. Marvin) Ok. Very good. What’s your name?
(Female voice) Sally.
(Dr. Marvin) Ok Sally. And how long have you been listening?
(Sally) I have been listening off and on for probably a couple months.
(Dr. Marvin) A couple months, ok. Well good because I talk about braces and orthodontics because I can make this a really long winded question and I could talk days about just orthodontics in general. And you said that your child, your 16 year old needs braces, right? Is this from; is that what he is saying? Or is this what you’re saying or is this what a dentists or orthodontist says?
(Sally) That’s from the orthodontist what he had said.
(Dr. Marvin) Ok. Obviously an orthodontist, ok if you go to a dentist they want to do dentistry, you do to an orthodontist they want to do braces if you want to go to endodontic they want to do root canals, you go to a Plummer they are going to look for plumbing stuff, ok. So first of all they have them sland they have a different sland, ok? So what’s traditional is that, and I am one of those in the past, I had braces as a teenager. And the reason is because that’s what normal is. Everyone else has teenagers and this is what is tradition, Ok. So that’s what dentists are taught and that’s what dentist recommends. When I say dentist I include orthodontist. Now let me ask you this, does your body continue to grow after being a teenager?
(Sally) Well, I mean it doesn’t usually most of the growth but I know that cells keep regenerating themselves and
(Dr. Marvin) Ok. I am going to tell you
(Sally) to replace.
(Dr. Marvin) You ever seen an eighteen year old continue to grow, like have what they call a late growth spurt and they get taller and bigger, you know until there are 21, 22 years old?
(Sally) I don’t know actually.
(Dr. Marvin) Ok. We’ll let me tell you this. Orthodontist and dentist are recommending these treatments because that’s that they have done in the past and that is what they know. Ok. It’s one of those things they know what they know and they don’t know what they don’t know. Ok.
(Sally) Right.
(Dr. Marvin) And what they are seeing they’re time with the patient ends when the braces come off and they put a retainer on but what they don’t see is what does this mouth look like? More importantly what does the jaw and the bite the rest of the body look like 20, 30, 40 years down the line. And most orthodontist and dentist do not take that into consideration. They don’t take that variable and that aspect in to consideration so what they’re doing is permanent.
(Sally) Right.
(Dr. Marvin) And what I am telling you is they don’t look in the future, all they care about is the end result when they take it off. And usually the end result looks nice. Ok. Because what they look at are teeth and gums. This is what all dentists look at, teeth and gums. They don’t look at what do things look in the future, they don’t look at what is underneath the teeth and gum which I look at, which I think is more important than the teeth and gums. I look at the whole body, so when they move teeth they are moving teeth at one point in time when I feel that the body still grows. Now most people have crowding, ok. Does your 16 year old have crowding in his mouth?
(Sally) Yes.
(Dr. Marvin) So what most dentist and orthodontist say is that your jaw is too small or your teeth are too big, or a combination of both because you have crowding, OK. So I am going to tell you that teeth size when it comes out, teeth don’t grow ok. They grow inside the bone and they continue to grow and they are growing to a predetermined size so that size does not change as you get older. But the jaw, the size of the jaw does change size so the bone where the teeth are in does grow. So when it grows, you know, it’s supposed to grown enough to hold in all your teeth all 32 of them. So what we need to do is we need to figure out why the jaw is not big enough, one of the reasons why it’s not big enough and one of the reasons why it’s not big enough is because we haven’t given it enough time for the jaw to grow.
(Sally) So you’re saying that people are getting orthodontia before their jaw is finished growing.
(Dr. Marvin) That is correct.
(Sally) I see.
(Dr. Marvin) Ok the other; there are many things that I could talk about. One of the reasons why it’s not growing is because the lounge which acts like an expander of your pallet, is not positioned in the right area for whatever reason so when it doesn’t push the pallet laterally or to the sides the jaw doesn’t grow and when the jaw doesn’t grow you see the end result which is crowding in the teeth. But the root cause of the problem is that the tongue is not in the right position to make the jaw bigger. So everything we do should be helping to promote the growth the jaw, ok. So that the teeth which are more passive are being put in the right position. Because I am going to fast forward if you do the orthodontics now, to correct the alignment of the teeth and the body still grows then the teeth are not going to be in the right position anymore, Ok. OR the jaw is going to be in the wrong position. You’re forcing the body to accept the position of the teeth when the body was still growing. Does that make sense?
(Sally) uh-hu.
(Dr. Marvin) Ok so humans, man, are getting in the way of nature. And when it does the jaw is too small or the jaw can’t grow because we are retaining the teeth to keep the jaw from growing and we don’t have enough space since the jaw is not big enough. We don’t have enough space to have the last teeth come in hence wisdom teeth don’t have enough room. They become impacted and dentist will recommend them being taken out preventively because you don’t have enough room or they are going to take it out when you do have a problem because you don’t have enough room. I am always asking what caused that or what’s next? And one of the reasons one of the reasons why I don’t like orthodontics is because of that scenario. There are so many things. I mean we didn’t even talk about the metal at all, ok. So you know just keep in mind that my views in orthodontics are in a minority just like everything else that I say. You need to take a common sense approach. You need to figure out what the more natural way figure out why the jaw is not growing. Figure out if your son is sensitive to the materials, we didn’t even look at toxicity as an issue. We need to figure out that all before making the decision to spend 5, 6, 7, 000 dollars on orthodontics, when it could be causing more dental problems in the future. You know, when you think you’re doing the best for your child now.
(Sally) It’s actually my daughter, and she does have one tooth, front tooth longer than the other and we don’t know if she was grinding. She says her jaw has never been sore but the orthodontist said that it did seem like her gum was even higher so that seems like that tooth hadn’t come in and orthodontics would help it. I mean, I don’t know. I bother her, so I don’t know if it would be helpful to, you know, one of the options of course the cheapest option would be to grind down the other tooth but then you’re like cutting away the enamel.
(Dr. Marvin) That is correct. You don’t want to do that. You definitely don’t want to do that. First of all, you can tell if there is wear or grinding or not necessarily grinding but if you have ware on a tooth, ok. But a restorative dentist or like a dentist like myself can tell if there is ware. You also have to look at grinding because that can be an underlying issue to the whole thing. And the bite in the wrong position. When I say bite I mean when you jaw closes and the trajectory that closes is it closing in the right trajectory? Ok.
(Sally) Actually she was told that her jaw is off a little bit, and she had played some sports and soccer and stuff so that they were thinking that her jaw was out of alignment because of that. And that is why only one tooth was warned away. That she probably was grinding somewhere at some point, but we just don’t know if she is still doing it. Like in her sleep, or anything.
(Dr. Marvin) So there is a lot more questions to get into play like why is the jaw out of alignment but does doing braces correct the alignment or does it make it worse or does it make it neutral? I mean there are so many variables I mean it’s incredible. I guess this is why I do dentistry, is because I love this challenge that there is its not set in stone. It’s not one size fits all even though every dentist wants to do it that way because it easier and cheaper that way. But there are so many aspects of it, you know teeth erupt ok. It’s called active eruption, and passive eruption. Ok. Your child is still going to grow and the gums are going too recited predictably, ok. So you don’t cut away gums and you don’t cut away natural enamel and you look at the big picture so you know, I would say put the brakes on. Get more information; schedule an appointment with us at the office to get an evaluation to get our opinion of what needs to be done, Ok. Before you go forward ok. Because unfortunately health care and capitalism don’t work well together but it is what it is.
(Sally) Yeah.
(Dr. Marvin) Every dentist, orthodontist included needs business and if you’ve had braces, I’ve had braces 3 times and jaw surgery. I mean I understand the business the business of it, and I understand the” you better do this or else” business aspect of it. And how they kind of get you, you want to do what’s best for your kids because you didn’t have it or what not, but you need to realize that you need to get more information and make a choice and you’re making a choice for your daughter and I am glad that you’re getting more information about this.
(Sally) Right. Ok.
(Dr. Marvin) Ok, so thanks for calling. Thanks for the question and call back anytime, and I look forward to seeing you in the office for that evaluation.

(Dr. Marvin) Thanks Sally. Great question about orthodontics. I mean I could go on 20 different tangents with that question that’s what we do. And if you like my answer, like I was tell you, we need to figure out the cause of the problems and when we figure out the cause of the problems sometimes it’s a lot easier and less expensive than going with the tradition route. So she is, Sally is going to save money and be more aware, more in tuned and her daughter is going to benefit from her extra knowledge from listening to the show and is going to provide her with a better foundation so that when she is 30, 40, 50, 60 years old she is not going to have the problems that I am trying to fix in my 30, 40, and 50 year olds, because their teeth and their jaw is in great alignment. The bite is good. The teeth are in their ideal position, the most natural position, the most God given natural position in relationship to the whole body. The teeth not in relationship with themselves. Hopefully that makes sense. It was a great call. I could go on forever. Thanks Sally.
